I'm guessing that it's a live snake that has crawled through the large piercing in that person's ear - y'know how some people will wear basically hollow metal tubes in their ears, and stretch them out to sometimes disconcerting sizes? Like that, but with a snake in it. Looks like a real snake to me, too. :) I think that's kinda cool.
